Introducing The Health Passport for People with an Intellectual Disability within Respite Services in St Michael's House
Overall Project Plan
Our plan is to introduce the Health Passport for People with an Intellectual Disability across our Adult Respite Services initially and then extend to other residential and day services to those who wish to have a passport (either a paper version or to use the App).
Two Project Coordinators from within St Michael's House will work alongside staff, service users and families to inform and support them when considering the adoption of a health passport and guide them through the process.

The project officially commenced in September 2022 and a number of information resources are currently being created to support different aspects of the project - they will appear here as we publish them and make them freely available to all.

We will be creating Easy Read documents and Videos to support the project.
